A painful heel on the first steps of the morning, an elbow that hurts when lifting a kettle, or a shoulder that prevents comfortable sleep can make treatment decisions feel urgent. Shockwave or steroid injections are both established options, but they do very different jobs. The right choice depends on the tissue involved, how long symptoms have lasted, your activity demands and what a detailed assessment shows.
For many tendon problems, the aim is not simply to reduce pain for a few days. It is to restore the tendon or joint’s capacity to cope with work, exercise and everyday movement. That usually means combining any procedure with a targeted rehabilitation plan.
Shockwave or steroid injections: the key difference
Shockwave therapy uses acoustic waves delivered through the skin to stimulate a biological response in painful tissue. It is commonly considered for persistent tendon-related conditions, particularly where symptoms have not settled with an appropriate period of load management and physiotherapy. The treatment is designed to support tissue remodelling and reduce pain sensitivity over time.
A steroid injection, often called a corticosteroid injection, delivers anti-inflammatory medication into or around a joint, bursa, tendon sheath or another targeted area. It can settle inflammation and reduce pain relatively quickly. In some cases, a local anaesthetic is used alongside the steroid.
Put simply, shockwave is usually chosen to support longer-term improvement in certain chronic tendon conditions. Steroid injections are more often used when inflammation is a major driver of symptoms and short-term pain relief may help someone move and rehabilitate more effectively. Neither option is automatically the best treatment because a scan result, pain location or diagnosis label alone does not tell the full story.
When shockwave therapy may be the better fit
Shockwave therapy is frequently used for persistent plantar fasciopathy, tennis elbow, calcific shoulder tendinopathy and some Achilles or patellar tendon problems. These conditions can involve changes in tendon structure and sensitivity rather than straightforward inflammation. For that reason, repeated steroid injections are not usually the preferred approach for many longstanding tendon complaints.
Treatment is normally delivered as a course, often over three to six sessions depending on the condition and response. It can be uncomfortable during application, especially over a very tender area, but intensity can be adjusted. Some people experience temporary soreness or a short-lived increase in symptoms afterwards.
Improvement is rarely instant. A gradual change over several weeks is more typical, and it may take up to 12 weeks to judge the full response. This slower timeline can be frustrating if you are trying to get back to sport or a demanding job quickly, but it reflects the fact that tendon recovery requires progressive loading, not only pain suppression.
Shockwave is not suitable for everyone. Your clinician will consider factors such as pregnancy, a history of blood-clotting problems or anticoagulant medication, an active infection, certain neurological conditions and the precise treatment area. A full assessment is essential before starting.
Why rehabilitation still matters after shockwave
Shockwave is not a replacement for strengthening. A calf-loading programme for heel pain, for example, helps the tendon and surrounding muscles tolerate walking, stairs and running. For elbow pain, grip, wrist and shoulder strength may all need attention.
Your physiotherapist will also look for aggravating habits that keep the problem active, such as a sudden rise in running distance, poor recovery between gym sessions, prolonged standing at work or an unsuitable return to manual duties. Adjusting these factors gives shockwave therapy a better chance of producing a meaningful result.
When a steroid injection may be appropriate
Steroid injections can be particularly useful for inflammatory joint or soft-tissue problems. Examples include painful shoulder bursitis, some forms of osteoarthritis, trigger finger, De Quervain’s tenosynovitis and certain joint flare-ups. They may also be considered when pain is severe enough to prevent sleep, work or participation in rehabilitation.
The attraction is understandable: some patients notice relief within a few days, although the response varies and the local anaesthetic can create a brief early improvement that does not represent the steroid’s full effect. Relief may last weeks or months, but it is not guaranteed and does not correct every underlying cause of pain.
A successful injection can create a useful window for physiotherapy. If shoulder pain has made it impossible to lift an arm or complete basic exercises, reduced pain may allow movement and strength to return. The value comes from what happens next – an appropriate rehabilitation plan, gradual activity progression and a clear understanding of how to avoid another flare-up.
Steroid injection risks and limitations
Steroid injections are safe for many people when used appropriately, but they are medical procedures with potential side effects. These can include temporary pain after the injection, skin thinning or colour change at the injection site, facial flushing and, for people with diabetes, a temporary rise in blood glucose levels.
The location matters greatly. Repeated steroid injections around certain tendons can weaken tissue and raise the risk of rupture. This is one reason clinicians are cautious around the Achilles tendon and do not use steroid injections as a routine answer to chronic degenerative tendinopathy. Injections may also be delayed or avoided if there is infection, poor wound healing, particular medication considerations or a planned operation.
Your clinician should discuss expected benefits, alternatives and risks in relation to your individual diagnosis before you consent. If an injection is recommended, image guidance may be used where appropriate to improve accuracy for particular structures.
The decision is based on diagnosis, not preference alone
It is easy to ask, “Which is better?” but that question can lead to the wrong decision. A person with a swollen, painful bursa may benefit more from an injection than shockwave. Someone with six months of plantar heel pain, reduced calf strength and a clear pattern of load-related symptoms may be better suited to shockwave alongside progressive exercise.
There are also situations where neither is the first step. A recent muscle injury may need protection and graded rehabilitation. A suspected tear, fracture, nerve-related pain or significant loss of function may require further investigation or referral. Pain that is accompanied by fever, unexplained weight loss, a hot swollen joint, new numbness or sudden weakness should be assessed promptly.
A thorough physiotherapy assessment should establish what provokes the pain, how the area moves and tolerates load, whether there are signs of inflammation, and what outcome matters most to you. Returning to five-a-side football needs a different plan from managing stairs comfortably after surgery or completing a full day at a desk.
What to expect from a treatment plan

Avoid using early pain relief as permission to return immediately to heavy lifting, long runs or intense training. Feeling better is encouraging, but capacity often lags behind symptoms. A staged return gives the treated area time to adapt and reduces the risk of simply restarting the same cycle of irritation.
