What is Sciatica?

Sciatica is a symptom, not a diagnosis — it describes pain that radiates along the path of the sciatic nerve, which runs from the lower back through the hips and buttocks and down each leg. The pain typically affects one side of the body and can range from a mild ache to a sharp, burning sensation or excruciating pain.

Sciatica occurs when the sciatic nerve becomes compressed or irritated — most commonly by a herniated lumbar disc, spinal stenosis, or piriformis syndrome. Physiotherapy is the primary treatment recommended by NICE and is highly effective for the majority of sciatica cases.

Common Causes of Sciatica?

Our physiotherapists assess all potential causes during your initial consultation to identify the root of your condition.

Lumbar Disc Herniation

The most common cause — a bulging or herniated disc in the lumbar spine presses directly on the sciatic nerve root.

Spinal Stenosis

Narrowing of the spinal canal compresses the nerve roots, causing sciatica — more common in adults over 50.

Piriformis Syndrome

The piriformis muscle in the buttock compresses the sciatic nerve — causing sciatica without a disc problem.

Spondylolisthesis

One vertebra slipping forward over another can compress nerve roots and cause sciatic symptoms.

Facet Joint Problems

Arthritic or inflamed facet joints in the lumbar spine can irritate nearby nerve roots causing radiating pain.

Trauma

Direct trauma to the lower back or buttock can irritate or damage the sciatic nerve causing pain and neurological symptoms.

How long does sciatica take to resolve with physiotherapy?
Acute sciatica typically improves significantly within 4–6 weeks of physiotherapy. Chronic or severe cases may take 3–6 months. Most patients notice meaningful pain reduction within the first 2–3 sessions.
Can physiotherapy cure sciatica?
Physiotherapy is the most effective treatment for sciatica in the majority of cases. We address the root cause — whether disc, joint or muscle — to provide lasting relief. Around 90% of sciatica cases resolve without surgery.
Is bed rest good for sciatica?
No — prolonged bed rest is not recommended for sciatica. Gentle movement and physiotherapy-guided activity is far more effective. We’ll advise on optimal movement strategies from your first appointment.
 
Should I have an MRI before physiotherapy?
Not necessarily. Most sciatica can be accurately diagnosed clinically without imaging. An MRI may be recommended if you’re not improving as expected or if surgical consultation may be required.
 
Do you offer home visits for sciatica?
Yes — if your sciatica is too painful for you to travel to a clinic, our physiotherapists can visit you at home. This is available across all our clinic catchment areas.
